Instructor – Continuing Care

AHS is dedicated to Enhancing Care in the Community, helping people be healthy, well and independent in their homes and communities. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. Edmonton Zone Supportive Living (SL) is a Program that supports ECC and provides an integrated system of health and personal support services to clients based on assessed need. SL has an immediate opening for a Case Manager Educator (CME) within the Education and Practice Development portfolio. The CME facilitates, coordinates, and supports the implementation of projects and initiatives within the program, which may be influenced by orientation, professional development, continuing education, changes in policy, procedure, protocols, and technology. The CME acts as a role model and resource to staff for professional development, clinical competency, and case management skills. This position works with an interdisciplinary team and consults with managers to determine learning needs and operational requirements. Supports new staff and encourages best practice by group and one on one coaching and mentorship. Mentoring entails review of another staff members work and providing appropriate feedback and support to address any areas for improvement. Attends site visits, clinic visits and case conferences with staff. Provides leadership and expertise for nursing best practice. Chairs nursing meetings and committee meetings as required. Influences and supports a culture of professional growth. Assists with the assessment of education needs of staff and the identification of appropriate learning resources.

As an Instructor, you will be an expert practitioner and key educator. You will take a leadership role in the planning, coordination, implementation and evaluation of education for nursing staff including orientation, compulsory and continuing education, and clinical development. You will collaborate with management and internal partners to promote standards of clinical, educational and evidence informed practice. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP).

Strong computer skills (Microsoft Office) are necessary. Experience with development, implementation, facilitation and evaluation of education materials and programming is required. Proven exemplary communication, interpersonal and organization skills are essential. Demonstrated leadership and teamwork abilities, along with the ability to work effectively in a self-directed environment are vital. Travel among sites is required, thus must have a reliable vehicle. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet. Physically capable of performing nursing tasks which could include transfer of clients. The successful candidate will work independently in clients’ homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs.

Baccalaureate degree in nursing is preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Post graduate studies or certification in adult education, gerontology, chronic disease management and/or palliative care would be an asset. Minimum 3 years nursing experience and/or clinical educator experience in a community setting preferred. Current working knowledge of Meditech, RAI, and Pixalere an asset.
